Medicare Summary Notice (MSN)

  • Why did I get it?
    You have Original Medicare and got medical services or supplies during the last 3 months.
  • When do I get it?
    Every 3 months, if you got medical services or supplies during that 3-month period.
You can also log into (or create) your secure Medicare account to sign up for electronic MSNs. If you sign up, you’ll get an email with a link to your MSN each month instead of waiting 3 months for a paper copy in the mail.


Medicare premium bill

  • What’s the notice called?
    CMS-500 Form
  • Why did I get it?
    You pay Medicare directly for your Part A premium, Part B premium, and/or Part D IRMAA.
  • When do I get it?
    On or about the 10th of each month.


Medicare Easy Pay premium statement

  • What’s the notice called?
    CMS-20143 Form
  • Why did I get it?
    You set up recurring premium payments through Medicare Easy Pay. Your premium statement includes the total amount we’ll deduct from your checking or savings account that month, along with a breakdown of that amount.
  • When do I get it?
    On or about the 10th of each month.


Change in plan availability (from Medicare)

  • What’s the notice called?
    CMS Part C Non-renewal Reminder Notice 
  • Why did I get it?
    Your current Part C plan is leaving the Medicare program, and you need to choose a new plan for the upcoming year.
  • When do I get it?
    November


Change in plan availability (from your plan)

  • What’s the notice called?
    Plan Non-renewal Notice
  • Why did I get it?
    Your Medicare Advantage Plan is leaving the Medicare program, and you need to look for a new plan for coverage in the upcoming year.
  • When do I get it?
    October


Drug coverage reminder

  • What’s the notice called?
    CMS Non-renewal Action Notice
  • Why did I get it?
    Your current Medicare drug coverage (Part D) has ended because your plan left the Medicare program. To continue your drug coverage, you need to choose a new Medicare drug plan soon, if you haven’t already.
  • When do I get it?
    January


Change in Social Security payment

  • What’s the notice called?
    Social Security Benefit Rate Change Notice
  • Why did I get it?
    Your Social Security payment amount is changing for the upcoming year.
  • When do I get it?
    December


Initial Income-Related Monthly Adjustment Amount (IRMAA) notice

  • Why did I get it?
    You have Medicare Part B and/or Part D coverage, and Social Security determined that IRMAA applies to you.
  • When do I get it?
    Any time


IRMAA adjustment notice

  • Why did I get it?
    Your Part B or Part D premium for next year is changing because your income changed.  
  • When do I get it?
    November

  • Why did I get it?
    To tell you about upcoming changes in your Medicare health or drug plan’s coverage, costs, and more.
  • When do I get it?

Part D Explanation of Benefits (EOB)

  • Why did I get it?
    To give you a summary of your Medicare prescription drug claims and costs for the last month.
  • When do I get it?
    Monthly, as long as you filled at least one prescription in the last month.


Notice of Creditable Coverage

  • Why did I get it?
    To tell you that your drug coverage from an employer/union or other group health plan is considered “creditable,” which means it provides the same value as Medicare Part D.
  • When do I get it?
    September


End-Stage Renal Disease (ESRD) coverage ending soon

  • What’s the letter called?
    ESRD coverage with Medicare ending soon
  • Why did I get it?
    Your Medicare coverage based on ESRD will be ending soon, and you need to take action to get other health coverage.
  • When do I get it?
    A few months before your Medicare coverage based on ESRD ends.
