Improving functional abilities is an important goal for LTCH patients. Each facility electronically submits data about patients' functional abilities to the Centers for Medicare & Medicaid Services (CMS) using the LTCH Continuity Assessment Record and Evaluation (CARE) Data Set.

Spontaneous Breathing Trials (SBT) are used to identify patients who are ready to breathe without a ventilator (mechanical ventilation). Assessing patients on ventilators to determine their need for SBT and performing SBT for patients who are ready for them are important processes for LTCH patients.

Each facility electronically submits data on these to the Centers for Medicare & Medicaid Services (CMS) using the LTCH Continuity Assessment Record and Evaluation (CARE) Data Set (LCDS).

Medication management

Medication management measures provide information on facility practices to reduce medication-related errors that may affect quality of life and/or other patient complications. Each facility electronically submits data about medication management facility practices to the Centers for Medicare & Medicaid Services (CMS) using the Long-term Care Hospital (LTCH) CARE Data Set form.

Discharge to community rates show the rate at which patients returned to home or community from the LTCH and remained alive without any unplanned hospitalizations in the 31 days following discharge from the LTCH. The data comes from Medicare enrollment and claims data.

This measure shows whether Medicare spends more, less, or about the same on an episode of care for a Medicare patient treated in a specific LTCH compared to how much Medicare spends on an episode of care across all LTCHs nationally. This measure includes all Medicare Part A and Part B payments made for services provided to a patient with Original Medicare during an episode of care, which begins on the day of LTCH admission and ends 30 days after discharge from the LTCH.
  • The data comes from Medicare enrollment and claims data.
  • The result is a ratio calculated by dividing the amount Medicare spent per patient for an episode of care initiated at this LTCH by the median (or middle) amount Medicare spent per episode of care nationally.
    • A ratio equal to the national average means that Medicare spends ABOUT THE SAME per patient for an episode of care initiated at this LTCH as it does per episode of care across all LTCHs nationally.
    • A ratio that is more than the national average means that Medicare spends MORE per patient for an episode of care initiated at this LTCH than it does per episode of care across all LTCHs nationally.
    • A ratio that is less than the national average means that Medicare spends LESS per patient for an episode of care initiated at this LTCH than it does per episode of care across all LTCHs nationally.
